  1. Print it and check every line.

  2. Sign it on the signature line.

  3. Add the document that proves the change, the original or an officially verified copy: your lease or other proof of accommodation for a new address, your new passport, a marriage certificate.

  4. Take it to an Interior Ministry office, or post it, send it from a data box (datová schránka) or e-mail it with a recognised electronic signature.
